ACTION East provides Assertive Community Treatment (ACT) and Intensive Case Management (ICM) services for adults in East San Diego County living with serious mental illness who need intensive community-based support to remain stable and successful in the community. The program serves individuals who may be experiencing homelessness, frequent psychiatric hospitalizations, involvement with the justice system, challenges engaging in traditional outpatient treatment, or other complex behavioral health needs. Services are delivered by a multidisciplinary team in homes, hospitals, shelters, and other community settings and include mental health treatment, psychiatric care, care coordination, supportive housing services, rehabilitation, crisis intervention, benefits advocacy, substance use support, peer support services, and connections to healthcare, employment, education, and community resources.
